The Digital Millennium Copyright Act (DMCA) established a process for resolving claims of copyright infringement. DMCA Report Requirements
- A description of the copyrighted work that you believe is being infringed.
- A description of the material that you claim is infringing and that you want to remove or access to which you want to disable, and the URL or other location of that material.
- Your name, title (if an agent), address, phone number and email address.
- A statement that you agree with the authority: "I have a good faith belief that the use of the copyrighted material about which I am complaining is not authorized by the copyright owner, its agent, or the law." authorized (e.g., fair use)"
- A statement that you are not lying: "The information in this notice is accurate and, under penalty of perjury, I am the owner, or authorized to act on behalf of the owner, of the copyright or rights monopoly allegedly violated".
- An electronic or physical signature of the copyright owner or person authorized to act on behalf of the owner.
